Are you preferred or contracted with my insurance?
We are contracted with many insurance companies; Aetna, Blue Cross, Blue Shield, CareAdvantage, Cigna, Great West, HealthNet, Medicare, PHCS, Prudent Buyer Plans, Triwest/Tricare, United Healthcare, Workers Compensation, in addition to many other insurances including auto. We strongly urge you to contact your insurance company directly or online to guarantee coverage.

What should I bring on my first visit?
Please be on time for your first scheduled visit. A late arrival may cause a rescheduling of your appointment. Please bring the following items to your first visit:

1) The referral or prescription written by your doctor

2) Health insurance card or

3) Workman's Compensation information which includes:

a) Claim number

b) Claim adjuster’s name and phone number

c) Date of injury

What type of clothing should I wear?
You should wear loose fitting clothing.  For example, shorts, T-shirts, sweat pants, or athletic shoes.

Will the same physical therapist see me?
Our goal is to keep you with the same therapist so your treatment will be consistent which is more beneficial for a speedy recovery.

How long will my appointments be?
The first appointment is approximately 1 hour which includes an Initial Evaluation and treatment. Each subsequent appointment takes between 30 and 45 minutes.
