My First Visit

Your first appointment is about one hour long. If you are unable to download the needed paperwork from this website through the links below, we ask that you arrive 15 minutes early to fill it out. Please be on time for your first scheduled visit. A late arrival may cause a rescheduling of your appointment. Remember the following items for your first visit:

1)    The referral or prescription written by your doctor.
2)    Health insurance card or  Workman's Compensation
       information which includes:
       a)  Claim number
       b)  Claim adjuster’s name and phone number
       c)  Date of injury


Registration Forms

Please fill out the following forms as completely as possible before your first appointment. The information helps the therapist set the most appropriate goals and treatment program for your problem. Bring the completely filled out forms to your first appointment along with your insurance card and referral from your doctor.

Are you preferred or contracted with my insurance?
We are contracted with many insurance companies; Aetna, Anthem Blue Cross, Blue Shield, CareAdvantage, Cigna, Great West, HealthNet, Medicare, PHCS, Prudent Buyer Plans, Triwest/Tricare, United Healthcare, Workers Compensation, in addition to many other insurances including auto. We strongly urge you to contact your insurance company directly or online to guarantee coverage.


What should I bring on my first visit?

What type of clothing should I wear?

You should wear loose fitting clothing.  For example, shorts, T-shirts, sweat pants, or athletic shoes.


Will the same physical therapist see me?

Our goal is to keep you with the same therapist so your treatment will be consistent which is more beneficial for a speedy recovery. Visit our Staff Bios to find out more about each therapist.


How long will my appointments be?

The first appointment is approximately 1 hour which includes an Initial Evaluation and treatment. Each subsequent appointment takes between 30 and 45 minutes.


Driving Directions

From Hwy 101(north or south):
Take Hwy 92 east and get off at the Foster City Blvd. Exit. This is the last exit before the San Mateo Bridge. Turn left at the first stop light and cross Foster City Blvd. You will see McDonald's and Taco Bell on the left. Turn right at the first stop sign and make an immediate left turn into the parking lot. Park your car and walk between the two buildings. We are the office at the far corner of the court yard.

From East Hillsdale Blvd:
Go east on East Hillsdale Blvd. Travel 1 block past the intersection of East Hillsdale and Foster City Blvd, turn left at the first traffic light. This is Pilgrim Drive. Turn right into the second parking driveway. After parking your car walk between the buildings. We are the office in the far corner of the courtyard.

From San Mateo Bridge:
Take the Foster City Blvd. Exit. Stay to the right and turn right onto Foster City Blvd. Get into the far left lane and turn left onto Triton Dr. at McDonald's restaurant. Then turn right at the stop sign. This is Pilgrim Drive. Turn left into the first parking lot. Park your car, walk between the 2 buildings. We are the office in the far corner of the courtyard.
